Statin therapy after acute myocardial infarction: are we adequately treating high-risk patients?

چکیده

After acute myocardial infarction, patients remain at high risk for recurrent cardiovascular events and mortality. Despite the compelling scientific and clinical trial evidence that lipid-lowering medications reduce mortality in patients after acute myocardial infarction, this life-saving therapy continues to be underutilized. A number of studies in a variety of clinical settings have documented that a significant proportion of patients after myocardial infarction are not receiving treatment with lipid-lowering medications when guided by conventional care. It has recently been demonstrated that implementation of a hospital-based system for initiation of statins prior to hospital discharge results in a marked increase in treatment rates, improved long-term patient compliance, more patients reaching low-density lipoprotein levels of less than 100 mg/dL, and improved clinical outcomes. Adopting in-hospital initiation of lipid-lowering medications as the standard of care for patients hospitalized with acute myocardial infarction could dramatically improve treatment rates and thus substantially reduce the risk of future coronary events and prolong life in the large number of patients hospitalized each year.

The Frequency of Cerebral Microembolism in Acute Myocardial Infarction

ABSTRACT Introduction: Stroke is more common in patients with cerebral microembolisms. Frequency of cerebral microembolisms (high intensity transient signals, HITS) in acute myocardial infarction has been reported about 17%. The factors that influence on microembolism after myocardial infarction (MI) are not definitive. Type of MI, Ejection fraction, Hx of Streptokinase is the factors that were...

Association of statin use and stress-induced hyperglycemia in patients with acute ST-elevation myocardial infarction

BACKGROUND Only a few information is available on the risk of stress hyperglycemia following acute myocardial infarction after statin use. We investigate the association of stress-induced hyperglycemia following statin use in patients with acute myocardial infarction. METHODS An observational analysis of 476 consecutive patients who suffered acute myocardial infarction was carried out. All se...

QT-Dispersion as a potential marker in prognosis of acute myocardial infarction

Introduction: QT dispersion in a 12 lead ECG represents the heterogeneity of ventricular repolarization. The prolongation of QT-interval dispersion increases the risk of coronary heart disease, ventricular arrhythmia, and sudden cardiac death in patients with myocardial infarction. We aimed to investigate the prognostic value of QT-dispersion in patients with acute myocardial infarction. Meth...

Patterns of statin initiation, intensification, and maximization among patients hospitalized with an acute myocardial infarction.

BACKGROUND Intensive statins are superior to moderate statins in reducing morbidity and mortality after an acute myocardial infarction. Although studies have documented rates of statin prescription as a quality performance measure, variations in hospitals' rates of initiating, intensifying, and maximizing statin therapy after acute myocardial infarction are unknown. METHODS AND RESULTS We ass...
